The UK Health Security Agency (UKHSA) has issued an urgent alert to British holidaymakers following a notable increase in reported cases of cyclosporiasis, a debilitating intestinal illness. This surge, with 67 cases identified since April 2026, predominantly affects individuals returning from popular tourist destinations in Mexico, particularly the vibrant coastal areas of Riviera Maya and Cancún. The warning comes amidst a much larger, ongoing outbreak in the United States, which has affected over 18,000 people across 45 states, though a definitive source for that outbreak remains elusive. This situation highlights the persistent global challenge of foodborne parasitic diseases and the imperative for heightened vigilance among international travellers.

Cyclosporiasis is caused by Cyclospora cayetanensis, a microscopic parasite that infects the small intestine. Unlike many bacterial or viral pathogens, Cyclospora requires a specific environmental stage, known as sporulation, to become infectious after being shed in faeces. This process typically takes days to weeks, meaning direct person-to-person transmission is extremely rare, as stressed by experts. Instead, the parasite spreads through the consumption of food or water contaminated with sporulated oocysts. Fresh produce, such as salads, herbs, and soft fruits, are frequently implicated in outbreaks because they are often eaten raw and can be easily contaminated through irrigation water, washing practices, or handling by infected individuals. The parasite’s resilience to common disinfectants like chlorine further complicates control efforts in food processing.

The scale of the current UK situation is concerning, with the UKHSA reporting 67 cases between April and mid-July 2026. A detailed breakdown reveals 30 cases in England, 27 in Scotland, and 10 in Wales during the period from April 30 to July 15, 2026. This distribution across the UK underscores the widespread nature of international travel and the potential for imported infections to affect various regions. Critically, of the 52 cases for which travel information was available, a striking 48 individuals had travelled to Mexico. Large-scale catering operations, buffet-style service, and the sourcing of diverse fresh produce from various suppliers can create complex pathways for contamination if hygiene protocols are not rigorously maintained at every stage, from farm to fork. The perceived safety and controlled environment of such resorts can also lead holidaymakers to lower their guard regarding food and water choices.

The symptoms of cyclosporiasis are profoundly unpleasant and debilitating. The hallmark symptom is severe, watery diarrhoea, which can be explosive in nature and occur with alarming frequency, sometimes up to 20 times a day. Beyond the gastrointestinal distress, infected individuals often experience a constellation of other symptoms, including loss of appetite, significant weight loss, abdominal cramps or pain, bloating, increased flatulence, nausea, and general fatigue. Some patients also report flu-like symptoms, further compounding their discomfort. Nick Palmiter, a 29-year-old from Monroe County, Michigan, who was affected by the US outbreak, vividly described experiencing "constant pressure and bubbling" in his stomach, likening the sensation to having "a balloon in your stomach." The prolonged nature of the illness, which can last for weeks or even months if untreated, significantly impacts quality of life and can lead to severe dehydration and nutritional deficiencies.

While most mild cases of cyclosporiasis can eventually resolve on their own, often over several weeks, antibiotics are typically prescribed for more severe or prolonged infections. The standard treatment involves trimethoprim-sulfamethoxazole (commonly known as Cotrimoxazole or Bactrim), which effectively targets the parasite. Prompt medical attention and accurate diagnosis are crucial, as misdiagnosis can lead to inappropriate treatment and prolonged suffering. It is imperative for individuals experiencing symptoms after international travel to seek medical advice and explicitly inform their healthcare professional about their travel history, enabling accurate diagnosis and appropriate management.

Preventative measures are paramount for travellers, especially when visiting regions where cyclosporiasis is endemic or outbreaks are reported.
